Central Coast Health Alliance Initiative Pitch Grants EOI

The Central Coast Health Alliance (the Alliance) refers to the collaborative relationship between the Hunter New England and Central Coast Primary Health Network (PHN) and the Central Coast Local Health District (CCLHD). This Expression of Interest (EOI) is for innovative initiatives that can be delivered via a collaboration between primary and secondary/tertiary care. From the EOI, shortlisted applicants will be invited to present at a pitch event to compete for sponsorship and seed funding for implementation of their initiative. 

Please refer to the Guidelines before completing the application. 

Submitting an application is no guarantee of funding.  For applicants who are invited to pitch, there is NO GUARANTEE of a minimum funding amount being awarded.

Conflict of Interest

Due to the nature of the collaborations,we acknowledge that there will be a level of perceived conflict of interest. 

Please declare if there are any additional conflicts outside of what would be considered as reasonable due to the working relationship between the LHD and PHN.

This may include personal or financial interests in any third parties involved in the project, suppliers, other partnerships, etc.

I confirm that I as the applicant: 

• has read and understands the submission guidelines  

• can meet the Pitch Night event requirements and can attend the local pitch event and coaching sessions (refer to guidelines for key dates)
